Create a lasting memory by including a photograph of your graduate (Kindergarten, high school, college, military, etc.) in a special advertising supplement that will publish on Wednesday, June 12th in the Life + Neighbors section.

You have several options:

• POSTAL MAIL: Complete the mail-in form (download it here) by Wednesday, May 30. Mail it, your photograph and your check for $20 per photograph, made payable to the Lexington Herald-Leader, to: 2013 Graduate Salute, Lexington Herald-Leader, 100 Midland Ave., Lexington, KY 40508-1999.
  
  
• DROP OFF: Deliver your photo and your $20 check made payable to the Lexington Herald-Leader, to the Lexington Herald-Leader front lobby, 100 Midland Ave, Lexington. Front lobby hours are 8:30 to 5:00 weekdays.
  
  
• E-MAIL: Include all of the information below in your email. Scanned photos must be in JPEG format. Email information and photo to grads@herald-leader.com. Provide your daytime phone number and we will contact your for billing information, or call (859) 231-1460.
  
  
• ONLINE: Fill out the form below and you will be sent an e-mail with a unique identifying number for your ad. You can mail in the payment (make checks out to Lexington Herald-Leader and mail to 2013 Graduate Salute, Lexington Herald-Leader, 100 Midland Ave, Lexington, KY 40508 or call (859) 231-1460 with credit card information. Please provide the ad id number in any correspondence to ensure prompt and accurate accounting.
  
  

The complete form, photograph and payment must be received by final deadline of noon Wednesday, May, 30. Complete one form per child. Do not send cash. Please include a self-addressed, stamped envelope if you wish to have your photograph returned after publication.
