State health officials are asking Kentuckians to take a survey about the H1N1 vaccination programs and the availability of the vaccine.
The Centers for Disease Control and Prevention is paying for the $17,500 effort.
The survey, at http://healthalerts.ky.gov, is anonymous. It will be available through Dec. 19. A phone survey also is being conducted.
Public health commissioner William Hacker urged those who are at high risk for getting the disease — including families with children, college students and other young adults, pregnant women and those with chronic health conditions — to take the survey.
