A fund has been set up at a Lexington bank for the family of a 5-year-old boy who was killed Sunday in a head-on collision Woodford County that allegedly was caused by a wrong-way driver.

Fayette County schools spokeswoman Lisa Deffendall said the family of Blake Cohorn, a kindergarten student at Harrison Elementary in Lexington, has medical, funeral and rent expenses.

Blake's parents, Wendi and Rocky Cohorn, were injured in the wreck, which occurred Sunday when they were returning from a weekend trip to Indiana. Wendi Cohorn was released from the hospital Wednesday, but Rocky Cohorn remained at University of Kentucky Chandler Hospital in serious condition Thursday.

Versailles police said Alfonso Diaz-Diaz, 26, of Louisville was driving a Volkswagen Jetta that collided with the Cohorn family's 1999 Chevrolet Lumina just before 10 p.m. Sunday. Diaz-Diaz was driving west in the eastbound lanes of U.S. 60 about a mile west of Versailles.

Diaz-Diaz was charged Monday with one count of second-degree manslaughter, five counts of first-degree assault, one count of first-degree wanton endangerment, one count of driving under the influence of alcohol with aggravating circumstances, and one count of driving without a license.

Donations should be sent to The Blake Cohorn Trust at Fifth Third Bank, account number 9380835729. The bank is at 1580 Leestown Road, Lexington, KY 40511.
